Enrollment in a gymnasium is carried out in accordance with the director’s order, which is issued on the basis of an application for enrollment in an educational institution from one of the child’s parents, to which are attached:

  • A copy of the child's birth certificate or a document certifying the identity of the education seeker (when submitting a copy in person, the original of the relevant document is presented). Children who have a certificate of registration of an internally displaced person (or one of their parents has the above-mentioned life expectancy) and who do not have a birth certificate will be enrolled in an educational institution without the relevant document. The educational institution will immediately inform the guardianship and trusteeship authorities at the child's place of residence about the absence of a birth certificate.
  • Document confirming the appropriate level of education
  • Personal matter
  • Original or copy of a medical certificate in the form of primary accounting documentation No. 086-1/o "Certificate of a student of a general educational institution on the results of a mandatory medical preventive examination", approved by order of the Ministry of Health of Ukraine dated August 16, 2010 No. 682.

Заява та вищезазначені документи надаються власноруч або направляються нарочним, або у форматі сканованих копій чи фотокопій, направлених факсом, електронною поштою (за вибором заявника).

The absence of a medical certificate as of May 31, 2025 is not a reason for refusal of enrollment.

The gymnasium takes measures to familiarize students and their parents or persons replacing them with the Statute, internal regulations, and other documents regulating the organization of the educational process.
